Abordagens terapêuticas na comunicação buco-sinusal: uma revisão de literatura.

Abstract

The bucco-sinus communication is considered a transoperative or postoperative complication, which may occur during exodontia of premolars and upper molars, destruction of the maxillary sinus by periapical lesions and removal of cysts or tumors of the palate or maxillary sinus, the communication occurs due to the proximity of the roots with the floor of the maxillary sinus. Although it is already considered a complication, it can lead to further complications, such as maxillary sinusitis or the formation of an oroantral fistula. Besides the drug treatment with the use of antibiotics, analgesics, anti-inflammatory and nasal decongestants, there is the treatment under surgical aspect that is determined by the size of the communication, all treatments are effective when chosen according to each situation. The surgical treatment is performed in order to close the oral sinus communication and must be carried out by a qualified dental surgeon. It consists of making a buccal or palatal flap, adding a fatty body (Bichat's ball) or bone grafts. The Caldwell-Luc technique can also be applied in this situation. The literature review was based on materials found in sites such as PubMed, Google Scholar and Scielo, and it was possible to conclude that all repair techniques are resolutive, but that the choice of which one will be more efficient will depend on the size of the communication, on the ability of the dental surgeon, on the time of diagnosis and whether or not there is the presence of infection.
